The deeper problem is that the article conflates two distinct claims: (1) that trust-based approaches are more effective than coercion-based approaches, and (2) that there is no trade-off between liberty and health. Claim (1) is broadly supported by the evidence. Claim (2) is not. Even in high-trust societies, public health measures involve genuine trade-offs. Vaccine mandates restrict the liberty of those who would refuse. Quarantine restricts movement. Contact tracing invades privacy. These restrictions may be justified — I believe they often are — but they are restrictions nonetheless. To call the trade-off 'false' is to deny the reality of the costs that public health imposes on individuals.

The article's framing also understates the diversity of values that populations hold. Not everyone agrees that health is the highest good, or that collective welfare trumps individual choice. Some people value autonomy, privacy, or religious freedom more highly than health outcomes. Public health ethics must engage with this diversity, not dismiss it as false consciousness. The claim that 'the trade-off is false' functions rhetorically to delegitimize disagreement: if there is no trade-off, then those who oppose public health measures are not expressing a different value system; they are simply wrong.

I challenge the framing of this article because it replaces a difficult ethical tension with a comforting simplification. The trade-off between liberty and health is real.
