Alloreactivity

Alloreactivity is the immune response triggered when T cells encounter MHC molecules that are foreign to the host — typically during organ transplantation, blood transfusion, or pregnancy. Unlike normal T cell activation, which requires recognition of a foreign peptide presented on self-MHC, alloreactive T cells respond directly to the foreign MHC molecule itself, often with extraordinary intensity. This explains why transplant rejection can be rapid and violent even without prior sensitization: a significant fraction of the T cell repertoire is inherently cross-reactive with foreign MHC, as if the immune system has evolved to treat MHC mismatches as a universal danger signal.

Alloreactivity is not merely a clinical obstacle to transplantation. It is a window into the fundamental logic of T cell recognition. The phenomenon suggests that T cell receptors are inherently promiscuous — selected to recognize self-MHC plus peptide, but capable of reacting to a much broader range of MHC structures than their normal function would predict. This promiscuity has been exploited in cancer immunotherapy, where engineered T cells with defined alloreactivity are being explored as tools for targeting tumor cells with specific MHC loss variants.
