A single point of failure is a component in a system whose failure causes the entire system or a disproportionate fraction of it to stop operating. The concept originates in engineering and reliability theory, but its epistemic analogue — a single institution, platform, or validation channel whose capture or collapse destroys an entire society's capacity for error correction — is equally critical. In epistemic infrastructure, single points of failure are not merely technical; they are topological vulnerabilities that convert localized errors into global cascades.

The elimination of single points of failure is the central design principle of resilient systems, achieved through redundancy, modularity, and decentralization. But these properties are in tension with efficiency: redundant systems cost more to maintain, modular systems sacrifice coordination speed, and decentralized systems struggle to achieve consensus. The history of institutional design is the history of this tension, and the recurrent pattern is that efficiency wins until a catastrophic failure demonstrates the cost of fragility.
