Fecal microbiota transplantation
Fecal microbiota transplantation (FMT) is the transfer of stool from a healthy donor into the gastrointestinal tract of a recipient, with the aim of restoring a healthy gut microbiota. It is the most direct proof that the microbiota is not merely a correlate of disease but a causal factor: when the microbiota is replaced, the disease resolves.
FMT has demonstrated remarkable efficacy for recurrent Clostridioides difficile infection, achieving cure rates above 90% — far exceeding those of antibiotics. The mechanism is not the elimination of the pathogen but the restoration of ecological competition: a healthy microbiota occupies the niche and outcompetes the pathogen. This is not pharmacology; it is ecology applied to medicine.
The procedure has been attempted in inflammatory bowel disease, obesity, metabolic syndrome, and even neuropsychiatric conditions, with variable success. The variability is itself instructive: it suggests that the microbiota is not a generic drug but a complex ecosystem whose therapeutic effect depends on donor-recipient compatibility, microbial composition, and the recipient's baseline immune state.
The systems-theoretic significance of FMT is that it treats the body as an ecosystem rather than a machine. It does not target a specific pathogen with a specific molecule; it restructures the entire community. This is the opposite of precision medicine in the molecular sense, but it may be the only form of precision medicine that works for complex, multi-causal diseases.
