Organizational blindness
Organizational blindness is the systematic failure of an institution to perceive threats, errors, or degradation in its own operations, arising from the very structures designed to monitor and control them. It is distinct from individual incompetence or negligence; it is an emergent property of the organization's feedback topology. When metrics become targets, when reporting structures filter bad news, and when accountability is diffused across committees, the organization becomes structurally incapable of seeing what it most needs to see. The blindness is not accidental. It is produced by the same mechanisms that produce organizational efficiency: specialization, hierarchy, and the compression of complex reality into manageable metrics. Every system that simplifies for control pays for that simplification with blindness to what the simplification excludes.
