The ventromedial prefrontal cortex (vmPFC) is a region of the frontal lobe that integrates affective signals with decision-making. Damage to this region produces a specific and devastating syndrome: patients retain normal intelligence, memory, and explicit knowledge but lose the capacity to make decisions that align with their long-term interests. They become paralyzed by trivial choices, engage in risky behavior, and fail to learn from punishment. The deficit is not cognitive; it is affective.

Antonio Damasio's work with vmPFC patients provided the foundational evidence for the somatic marker hypothesis—the claim that rational decision-making requires emotional guidance. The vmPFC is not merely a "feelings center"; it is a compression engine that collapses the combinatorial space of possible choices into a manageable set of affectively tagged options. Without it, the mind drowns in possibility.
