The article frames requirements failure as "the central error in most software projects" and builds the entire discipline around this premise. I challenge this framing as a consulting-industry narrative that systematically understates the severity and frequency of implementation failure.

The evidence cited — that requirements errors are "the most expensive to fix" — comes primarily from studies of business software projects conducted by consulting firms (IBM, Standish Group) in the 1980s-2000s. These studies measure "cost to fix" in terms of project budget overrun, not in terms of lives lost, systems compromised, or societal harm caused. When an ATM dispenses the wrong amount because of a requirements ambiguity, the bank loses money. When a Therac-25 radiation therapy machine delivers massive overdoses because of a race condition in the implementation, patients die. When Toyota's unintended acceleration bug causes sudden acceleration, the root cause was not that the requirements failed to specify "the car should not accelerate unexpectedly" — it was that the implementation of the throttle control system had stack overflow errors.

In safety-critical domains — avionics, medical devices, autonomous vehicles, industrial control — implementation failures dominate the incident record. The FAA's database of aviation software incidents, the FDA's MAUDE database of medical device failures, and the NTSB's investigations of automotive software failures all point to the same pattern: the requirements were often clear and correct; the implementation failed to satisfy them. The Ariane 5 explosion (1996) was not caused by ambiguous requirements. The requirements explicitly stated that certain alignment data should not be used after launch. The implementation reused code from Ariane 4 that violated this requirement.

Requirements engineering as a discipline has valuable contributions: stakeholder analysis, traceability, formal specification. But its central narrative — that building the wrong thing is worse than building the thing wrong — reflects the economics of commercial software development, where market fit matters more than safety. This is not a universal truth. It is a domain-specific observation that has been illegitimately generalized.

I propose an alternative framing: the "central error" is context-dependent. In market-driven software, requirements failure dominates. In safety-critical software, implementation failure dominates. In scientific software, both fail against the standard of reproducibility. Requirements engineering should recognize its own scope conditions rather than claiming to be the foundational discipline of all software development.
